THINGS TO KNOW BEFORE YOU COME IN

Did you know?

A routine eye exam and a contact lens exam are not the same.

  1. A Routine Eye Exam - ensures good, healthy vision by checking visual acuity, need for eyeglasses prescription, coordination of the eye muscles, front of the lens including the cornea, the iris and the interior and back of the eye. Your eyes will be dilated during the exam.

  2. A Contact Lens Exam - is a special exam in addition to your Routine Eye Exam. This special exam assess the curvatures of the eye, evaluates the cornea and eyelids in relationship to the contacts as well as the movement of the lens on the eye. This exam also determines your contact lens prescription, because it is not the same as the glasses prescription.

  3. Not All insurances cover these exams, nor do all insurances cover the exams on a yearly basis. Prior to your appointment you must contact your insurance provider to determine if one or both of these exams will be an out of pocket expense at the time of your visit.

  4. Our optical center offers a full range of services such as fulfilling any eyeglasses prescription, scratch resistant coating on plastic lenses, assistance with selecting your eyewear, and complementary cleanings at any time. Most insurances are accepted in our optical department. When visiting our optical center make sure you have your current insurance card and/or proper insurance forms along with the insurance holders information so we can processyour order immediately.


PLEASE BE PREPARED. PLEASE BRING THE FOLLOWING WITH YOU ON THE DAY OF YOUR APPOINTMENT:

  1. All current insurance cards

  2. Completed patient registration form

  3. Completed medical history form along with a list of medications and reason for medications for each condition.

  4. Your current eyeglasses

  5. If you wear contacts you must wear them into the office

  6. Sunglasses for dilated eyes

  7. obtain an insurance authorization, referral or eye exam form if required by your insurance provider.

 

  • CHILDREN AND YOUNG ADULTS UNDER THE AGE OF 18 YEARS MUST BE ACCOMPANIED BY A PARENT OR GUARDIAN UPON ARRIVAL AND DURING THEIR APPOINTMENT. IF NOT ACCOMPANIED THE APPOINTMENT WILL BE CANCELLED. 

  • ALL PAYMENTS ARE REQUIRED AT THE TIME OF YOUR VISIT.

  • YOU ARE RESPONSIBLE FOR KNOWING YOUR INSURANCE BENEFITS. IN THE EVENT OUR INSURANCE DEPARTMENT DOES NOT RECEIVE THE NECESSARY AUTHORIZATIONS, THE APPOINTMENT WILL NEED TO BE RESCHEDULED OR THE PATIENT WILL BE ASKED TO PAY FOR THE SERVICES PROVIDED.
